What is a Primary Care Network? 

A Primary Care Network consists of groups of general practices working together, and in partnership with community, mental health, social care, pharmacy, hospital and voluntary services in their local area. PCNs build on existing primary care services and enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home. Clinicians describe this as a change from reactively providing appointments to proactively caring for the people and communities they serve.

Network Clinical Pharmacists

Our team of Clinical Pharmacists work across the practices in the network to support those with long-term conditions like Asthma, COPD and Diabetes, and review patients taking multiple medicines to make sure their medication is working. 

Network Physician Associates

Our team of Physician Associates work across the practices in the network to examine, diagnose and treat a wide range of health conditions. 

Network Physiotherapists 

Our team of Physiotherapists work across the practices in the network to assess, diagnose and treat a range of complex muscle and joint conditions preventing the need for referrals to hospital.

Network Mental Health Team

Our Practices have chosen to prioritise Mental Health, as we are aware of the impact COVID-19 has had on our patients. In order to address this, we have used funding to employ an Advanced Practitioner in Mental Health and a Mental Health Practitioner who will be able to provide longer appointments for more comprehensive reviews, medication advice and support. 

Social Prescribing Link Workers 

Our Social Prescribing Link Workers work across the practices in the network to help patients work towards debt reduction, financial stability, better housing and employment conditions, and reduced social isolation. They also provide a wellbeing service that aims to combat loneliness and isolation in our community. Find out more about the work that our Citizens Advice Liverpool Link Workers do here.

Network Admin Team 

Our Admin team help to support the practices with administration tasks, as well as booking patients into our Additional Roles Clinics.
