We strive to be a Network that can provide a wide range of services for our patients.

Accessibility

  • Our Network Operations Team are making appointments more accessible to patients, by calling through and personally inviting each patient to book their appointment. This takes the onus off the patient, who may not have the same access to a phone or the internet as those more engaging patients, to ensure that they are seen for their Chronic Reviews.

Services

  • Mental Health - We have a large mental health team who are innovating and developing what mental health support in Primary Care looks like. Our team offer a range of appointments, from physical health checks and CBT style talking therapies, to longer appointments with our senior mental health team.
  • Sexual Health - Our LARC service has been designed to help patients access quicker appointments from practices close by to them. Becky has led the way with this service, becoming the first Pharmacist in the country to fit Implants, and soon to be the first Pharmacist to fit coils.
  • Women's Health - We have a dedicated number of clinicians across our PCN who are striving to improve Women's Health, including menopause support, pessaries, cervical screening and much more.
  • Cancer Screening - We are passionate about making sure our patients are well informed when it comes to cancer screenings, and engage with secondary care regularly to make access to appointments easier.
  • Care Homes and Housebound - Our dedicated Paramedic team are driving forward our housebound and care home service, helping to support practices and patients who cannot otherwise get to the surgery.

Inequalities

Because of our wide-ranging demographics, we want to make sure that our services reach every patient regardless of age, gender, ethnicity, or language spoken. We will do this by:

  • Engaging with Community Champions to make sure our approach is well informed for our different communities
  • Hold smaller, more focused events that are tailored to a specific patient population's need
  • Listen to feedback from patients, to help improve and develop services
