Important: Housebound Service
Home visits are available for patients who are housebound, meaning they are unable to come into the practice without assistance. If you are housebound and would benefit from home visits please contact your GP surgery.
The Housebound Team consists of three paramedics who help support practices by doing home visits and care home visits to patients who cannot otherwise get to the surgery.

Chronic Reviews
The paramedics carry out chronic reviews and routine checks such as:
- Asthma review
- COPD review
- Diabetes review
- Hypertension review
- LD reviews
- SMI Physical Health Checks
- Bloods and Urine sample
- BP and BMI
Referrals
They are able to refer to services that may benefit the patient such as:
- Podiatry
- Citizens advice
- Stop smoking services
- Mental Health Team
Immunisations
They administer vaccines such as:
- Shingles
- Covid-19, Flu
- Pneumococcal
- RSV
- B12 injections
They also assist with acute issues on GP request.
Enhanced Health in Care Homes
Our Enhanced Health in Care Homes (EHCH) team works with care home residents, their families and care home staff to provide personalised care, improve outcomes, and promote independence for people living in care homes.
We provide individual assessments and personalised plans for care home residents, which aim to keep residents in the best health possible, and ensure their health choices are respected.
Our team delivers the National Framework for Enhanced Health in Care Homes.
