Clinical Pharmacists

The PCN has six pharmacists who work across our GP practices, they have specialist expertise in the use of medicines, helping patients with their health and social needs by reducing inequalities in health care. Pharmacists can also help people to get the best from medications or supporting in managing long-term conditions, carrying out chronic reviews such as:

 

  • Asthma review
  • COPD review
  • Diabetes review
  • Medication review
  • Hypertension review
  • Cholesterol review

 

All Pharmacists are prescribers therefore if during your consultation you require a medication change, this can be arranged the same day.

 

For certain chronic reviews it is essential to have a blood test so the clinician can complete the review, if possible book your bloods in at your GP practice prior to Diabetes and cholesterol reviews. If not you will be given a bloods form to book this in after the assessment.
